Circa £47,000 per annum, plus generous performance bonus, up to 40% of salary

This contract is offered as 40 hours per week to include some weekends (usually one in four), occasional nights and share of the emergency ‘on-call’ support line, shared with the management team.

Colten Care are recruiting for a dynamic Clinical Lead (Frailty Lead /RGN / Senior Nurse) who can lead the management of clinical frailty in the nursing home.    

Our care home residents are living with moderate to severe frailty and your role will be to take the lead in ensuring that they receive outstanding clinical care. Leading the clinical team you will manage the complex clinical needs of the residents whilst also promoting their independence and well-being.

The scope of the role is diverse, requiring a flexible individual with a ‘can do’ approach. Responsibilities include leading and mentoring the clinical team to ensure timely escalation to emergency care services when needed, acting to reduce the risk of avoidable hospital admissions, as well as expert advance care planning at end of life.

The role is ‘hands on’ ensuring that whilst your leadership and management skills are supported to grow and develop, you will be able to maintain a high level of clinical skills practice, enjoy continuity of care and truly meaningful engagement with residents and their families.

Excellent communication skills are a fundamental part of the role as you'll regularly speak to residents, families, staff, outside agencies and the wider multidisciplinary team in a confident and professional manner.

You will be knowledgeable about the Care Quality Commission’s regulations and take an active role, in partnership with the Home Manager, to lead the home to meet the 'good' and 'outstanding' characteristics of the CQC key lines of enquiry.

In the absence of the Home Manager you will take the lead in the day to day running of the home.

About you

Ideally, you’ll be a Senior Nurse, Clinical Lead, Nurse Qualified Deputy Manager or Clinical Head of Care with experience in the management of frailty, looking to develop your career in older peoples care. You relate well to people at all levels, enjoying all aspects of nursing and managing teams. You’ll be someone who can inspire, motivate and lead by example.

This role will suit someone who is a strong leader, highly clinically skilled, well organised, able to take the initiative, has a positive outlook, enjoys a challenge, works hard and is motivated to make a positive difference to the lives of our residents.
